If your student has or will receive medication during their school day please make note, all medications (prescription and non-prescription) MUST be picked up by a parent, guardian, or parent-designated adult, by the last full day of instruction, and/or if the child transfers to another school. The school is unable to properly store medications over the summer. Medications that remain in the school clinic will be disposed of after dismissal, on the last full instructional day of school, May 22, 2019.
If you need medication forms for next year, please see the links to the forms. Please note that Epipens require the Allergy Emergency plan to be completed, Inhalers require the Asthma IHP Safety Plan to be completed, and Diastat requires the Seizure IHP Safety Plan to be completed. These 3 medications do not require the prescription form, only the appropriate IHP/Emergency plan form. If you student suffers from things such as migraines, IBS, urinary issues, etc, please use the generic IHP form that can be tailored to your student's needs. If your student simply needs to take a daily prescription medication, please use the prescription medication form.
