Dear Parent/Guardian,

As we come to the close of this challenging and unique school year, we want to remind you that if your student has medication in the school clinic it will need to be picked up by you, or a designated adult. Your school’s pick-up days/times are coordinated with the locker clean out days. Nurse Angie will be set up in front of the main entrance to the main building on Monday, May 11th from 12:00pm-2:00pm, Tuesday, May 12th from 5:00pm-7:00pm, and Thursday, May 14th from 12:00pm-2:00pm. Medication may not be sent home with students. Any medication not picked up by May 15th will be destroyed. If you are unable to make the times offered and need to arrange a separate time, email your school nurse as soon as possible at This email address is being protected from spambots. You need JavaScript enabled to view it.. Remember, you will need to sign the medication out at this time. We ask that in an effort to minimize contact, please bring your own pen with you.

If your child will attend Sumner County Schools next school year and, in an effort to maintain your child’s continuity of healthcare, take this opportunity to pick up health plans and prescription medication forms for the upcoming year. These forms must be completed annually and will need to be given to your school nurse prior to the first day of the 2020-21 school year. They will be available on the dates listed above for all students.
